You've spent the entire game in service to the One Who Waits. Now you must choose whether to kneel and completely offer yourself in sacrifice for them or stand and declare yourself as strong as a god. Here's what happens depending on which option you pick.
Kneeling to the One Who Waits in Cult of the Lamb
If you kneel to the One Who Waits, be prepared for a fairly anticlimactic end to a game you've spent hours working on. When you enter the Central Chamber that houses the One Who Waits, they will take away your duties of service and demand that you give up the Red Crown.
Should you submit to the One Who Waits, the Red Crown will float off the lamb and back to the One Who Waits before they levitate the lamb and drain your life.
After this, the game will end, and you will be directed back to the main menu.

Refusing to kneel in Cult of the Lamb
If you refuse to kneel to the One Who Waits, prepare for a four-part fight with two possible endings.
You will first fight Baal and Aym, the knights who stand alongside the One Who Waits for the entirety of the game. They each utilize move sets that mimic bosses from earlier in the game, so they're easy to dispatch as long as you focus on taking minimal damage.
After the knights are killed, you will square up against your former god. The first phase of this final boss fight features an extra meaty health bar for the One Who Waits and enhanced movement abilities. It's a good idea to brush up on some tips before you head into this fight.
Once you've beaten the One Who Waits in phase one, their true form will be unleashed. The arena will change and suddenly resemble a hellscape, and your cult members will be hanged from crosses on the sides of the battlefield. The One Who Waits will pluck out their eyes, transforming into minibosses.
This can be an overwhelming fight due to the sheer number of moving components to keep track of, but once you've finished, the One Who Waits will transform into a small, frightened animal. You will have to choose whether to spare or murder the animal.
Sparing the One Who Waits
Should you spare the One Who Waits, they will be teleported back to your camp like every other cult member and can then be converted into a full-on cultist. The One Who Waits comes with the immortal trait and will never die of old age.
Murdering the One Who Waits
If you select murder, the animal version of the One Who Waits will taunt the lamb and claim that you are no better than them. The lamb will then murder the One Who Waits, transferring the god's powers to the lamb.
