Even though the free Infant Update in The Sims 4 came with many new additions for babies, Changing Tables were not among them. This means you cannot get a Changing Table in The Sims 4 with just the base game. In all statements by EA regarding items given in the Infant Update, Changing Tables are not mentioned anywhere.

The only pack that is expected to feature Changing Tables is the Growing Together Expansion Pack. EA may decide to add more Changing Tables to other packs or Base Game in the future, but no confirmation has been given at this point.
